These Terms and Conditions ("Terms") govern your access to and use of our website (https://urnextlab.com), online appointment request tools, interactive diagnostic calculators, walk-in laboratory services, and mobile concierge phlebotomy services.

By accessing this website, submitting an appointment or inquiry form, or receiving clinical specimen collection and diagnostic testing services from UR Next Lab, you acknowledge that you have read, understood, and agreed to be bound by these Terms and our Privacy Policy & HIPAA Notice of Privacy Practices.

All content presented on this website—including test descriptions, specimen guides, fasting recommendations, turnaround estimates, and interactive test finder suggestions—is provided for educational and informational purposes only and does not constitute formal medical advice, clinical diagnosis, or physician treatment.

• No Physician-Patient Relationship: Utilizing our website, ordering direct-access wellness panels, or speaking with our phlebotomy staff does not establish a primary care physician-patient relationship.
• Clinical Interpretation: Laboratory test results must be interpreted in the context of your complete medical history, physical symptoms, and clinical presentation by a qualified physician, nurse practitioner, or physician assistant.
• Biological Variability: While UR Next Lab adheres to rigorous quality control protocols under federal CLIA standards, biological specimens are subject to pre-analytical variables (such as patient hydration, fasting compliance, medications, or hemolysis) that may occasionally require specimen recollection or confirmatory testing.

UR Next Lab operates as an accredited diagnostic testing facility holding federal Clinical Laboratory Improvement Amendments (CLIA) Certificate of Waiver #10D2347904 and National Provider Identifier (NPI) #1538076583.

• In-House Point-of-Care & CLIA-Waived Testing: Rapid infectious disease swabs (COVID-19, Influenza A/B, RSV, Strep A), rapid blood chemistries, urinalysis dipsticks, rapid HIV/STD screens, and point-of-care toxicology screens are performed directly inside our Jacksonville laboratory using FDA-cleared instrumentation.
• Reference Laboratory Send-Out Panels: Advanced hematology, comprehensive metabolic panels, endocrine assays, specialty hormone profiles, and confirmatory molecular NAAT/PCR panels may be collected by our certified phlebotomists, centrifuged and stabilized in-clinic, and dispatched via temperature-controlled courier to our partner CLIA/CAP-accredited reference laboratories.
• Direct-Access vs. Physician-Ordered Testing: Patients may present a valid requisition from their licensed healthcare provider or elect self-pay direct-access wellness and confidential screening panels where permitted by Florida law.

To ensure prompt clinical care for both in-clinic visitors and homebound/corporate mobile patients, the following scheduling terms apply:

• Walk-In Clinic Hours: Walk-in patients are welcomed Monday through Friday from 8:00 AM to 6:00 PM EST (closed daily from 12:00 PM to 1:00 PM for clinical sanitization and lunch).
• Saturday & Mobile Concierge Visits: Saturday hours (7:00 AM to 12:00 PM EST) and all Mobile Concierge Phlebotomy dispatches across Northeast Florida (Duval, St. Johns, Clay, and Nassau counties) are conducted strictly by pre-scheduled appointment.
• Mobile Dispatch Pre-Authorization: Prior to dispatching a mobile phlebotomist to a private residence, assisted living facility, or corporate office, patient demographics, clinical test selections, and payment authorization must be verified by our dispatch coordinator.
• Cancellations & Rescheduling: For mobile phlebotomy appointments, we kindly request at least 24 hours' advance notice for cancellations or address changes. Missed mobile visits where the phlebotomist has already arrived on-site may be subject to a standard mobile dispatch travel fee.

UR Next Lab is committed to 100% upfront healthcare price transparency with zero surprise facility fees or hidden administrative charges.

• Payment at Time of Service: All diagnostic testing, specimen draw fees, and mobile dispatch fees are due at the time of service (or prior to mobile phlebotomist dispatch). We accept Cash, Visa, Mastercard, American Express, Discover, and all Health Savings Account (HSA) and Flexible Spending Account (FSA) cards.
• Insurance & Itemized Superbills: To maintain fast turnaround times and low direct prices without third-party insurance delays, UR Next Lab operates primarily on a transparent direct-pay model. Upon completion of your visit, we gladly generate an official Itemized Insurance Superbill containing our CLIA ID (#10D2347904), NPI (#1538076583), and applicable CPT diagnostic codes so you may submit a claim directly to your health insurance carrier for potential out-of-network reimbursement.
• Refund Policy: Once a biological specimen (blood, urine, or swab) has been collected, processed, or consumed in diagnostic reagents, test fees are non-refundable. If a test is cancelled prior to specimen collection, any pre-paid diagnostic assay fee will be refunded in full.

Published turnaround times (such as 10–15 minutes for rapid Point-of-Care tests, Same-Day for in-house CLIA-waived tests, and 24–48 hours for reference panels) represent standard clinical averages.

While UR Next Lab achieves its published turnaround windows on the vast majority of specimens, turnaround times are good-faith estimates and cannot be guaranteed in cases where a specimen requires secondary dilution, confirmatory reflex testing (e.g., reactive HIV/Syphilis confirmation or non-negative drug screen GC/MS confirmation), or courier delays due to severe weather.

• Photo Identification Required: Every patient must present a valid government-issued photo ID (Driver's License, State ID, Passport, or Military ID) prior to specimen collection to ensure 100% patient-specimen matching.
• Pediatric & Minor Patients: Patients under 18 years of age must be accompanied by a parent or legal guardian who provides written informed consent, except where Florida statutory law explicitly permits confidential minor testing (such as certain sexually transmitted infection screenings).
• Workplace & Chain-of-Custody Drug Testing: Donors reporting for employer-mandated or chain-of-custody toxicology testing must adhere to strict collection room protocols, including emptying pockets, leaving outer garments and bags outside the restroom, and signing tamper-evident custody seals.

SMS Messaging Program Terms: By providing your mobile phone number and opting in to receive text messages from UR Next Lab and Services Solutions, LLC, you consent to receive SMS messages regarding appointment confirmations, mobile phlebotomy arrival notifications, diagnostic service updates, and relevant communications.

• Opt-Out (STOP): You can cancel the SMS service at any time. Simply text STOP to 1.833.487.6398. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us.

• Message Frequency & Rates: Message frequency varies. Message and data rates may apply for any messages sent to you from us and to us from you. Wireless carriers are not liable for delayed or undelivered messages.
• Privacy: If you have any questions regarding privacy or how we handle your SMS opt-in data, please read our Privacy Policy. We never share your phone number or SMS consent opt-in data with third parties or affiliates for marketing purposes.

Intellectual Property: All content, branding, logos, custom photography, diagnostic tools, code architecture, and visual designs on urnextlab.com are the exclusive intellectual property of UR Next Lab and Services Solutions, LLC and are protected by United States copyright and trademark laws.

To the fullest extent permitted by applicable Florida and federal law, UR Next Lab and Services Solutions, LLC, its clinical staff, officers, employees, and partner reference laboratories shall not be liable for any indirect, incidental, consequential, or punitive damages arising out of your use of this website, scheduling delays, or reliance on preliminary diagnostic screening information. In all events, our maximum aggregate liability relating to any specific laboratory test shall not exceed the amount paid by you for that specific diagnostic test.

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Florida, without regard to conflict-of-law principles. Any legal action or proceeding relating to these Terms shall be brought exclusively in the state or federal courts located in Duval County, Florida.
